  WARNING: DO NOT SCHEDULE AN APPOINTMENT. If you are having any of the following CHEST PAIN, SEVERE HEADACHE, NUMBNESS, WEAKNESS GO DIRECTLY TO THE ER. If you think you may need stitches please contact the office to make the appointment
• Wellness Visit-Child
  Physical ONLY- IMMUNIZATIONS CAN NOT BE GIVEN AT THIS LOCATION.
• Wellness Visit- Adult
  Basic Physical
• Well Women Physical
  Well Women Physical with Pap
• DOT Physical
  DOT physicals are only available to CMCSS or County Employees who are required to have one for their position in CMCSS or the County.
• Sports Physical
  Please fill out Sports Physical form and bring to visit. Forms are located on the CMCSS On-Site website
• Employment Physical
  Employment physicals which are required for CMCSS or County employees.
• Prescription Refill Appointment
  Prescription Refill- Please list medications which need to be refilled. If bloodwork is needed for refills an additional appointment may be required.
• Bloodwork- Outside Order
  Bloodwork ordered from a provider outside the On-Site Clinic. Patient must be order with them to appointment.
• Bloodwork-Inside Order
  Bloodwork ordered from a provider in the On-Site Clinic. Bloodwork must be ordered prior to visit. If you are requesting bloodwork, you must first schedule a wellness visit. Bloodwork may be done at wellness visit if time allows.
• Flu Shot
  Flu Shot- Only one patient can be scheduled for appointment. NOTE: The flu vaccine available at the On-Site clinics is available to School System Employees and dependents only. The County has elected to conduct a flu vaccine program separately
• Allergy Injection
  Allergy or B12 Injection
